Request a Transcript

Official Transcripts may be requested by the student in writing. This information can be obtained by calling (719) 384-6831. You may request as many copies of your transcript as you need for free. Follow the steps below to easily request your transcripts in writing:

1. Click here to open and print the transcript request form (you will need to have the free adobe acrobat reader installed on your computer. You can download this program at www.adobe.com)
   
   
2. Fill out the request form. You must include the name which you attended under, your social security number, the years you attended, the address where you want the transcript sent, and your signature.
   
   
3. Fax or mail the transcript request form to:
