Request a Transcript

Official Transcripts may be requested by the student in writing or completing the online form below. This information can be obtained by calling (719) 384-6831. You may request as many copies of your transcript as you need for free. Follow the steps below to easily request your transcripts:

Request a Transcript by Writing

1. Click here to open and print the transcript request form (you will need to have the free adobe acrobat reader installed on your computer. You can download this program at www.adobe.com)
   
   
2. Fill out the request form. You must include the name which you attended under, your Student S# or last four digits of your social security number, the years you attended, the address where you want the transcript sent, and your signature.
   
   
3. Fax the request to (719) 384-6933 or mail the transcript request form to:
